HealthView Services is committed to protecting your privacy and to making sure that you understand how your information is used. HealthView Services believes that only informed members and other website users can make informed decisions about their privacy. Please read this privacy policy carefully. It explains how we collect and share your personal information, and gives you options to limit how we share your information and communicate with you.
What it Does
This Privacy Policy describes, among other things, the types of information we collect about you when you join HealthView Services, visit our website, or participate in HealthView Services events and offerings. This policy also explains how your personal information, including your name, email address and other contact information, may be shared; how you can prevent the sharing of your personal information; and how your personal information is protected from unauthorized use.
Why We Collect
We collect information from you in order to:
- Deliver the information, programs, products or services you have requested from us
- Provide you with customer support
- Communicate with you by email, postal mail, telephone, and/or mobile devices about information, programs, products or services that may be of interest to you either from us or affiliates or providers
- Evaluate the effectiveness of HealthView Services benefits and offerings
- Display advertising tailored to your interests or background
- Perform data and statistical analysis
- Enforce our terms and conditions, and perform any other functions otherwise described to you at the time you provide your information
What We Collect
When you sign up with HealthView Services , we collect basic information such as your name, contact information, zip code, preferences and date of birth. We collect information about your participation in HealthView Services activities.. In the following sections, we explain in more detail the types of information we collect online.
When you visit our website we collect Personally Identifiable Financial Information (“PIFI”), defined as any information provided in order to obtain a financial product or service or information about the consumer resulting from any transaction between us and that consumer. We also collect information we receive from you during online registration and when you complete other forms.The confidentiality of PIFI is protected by Federal and state consumer privacy laws and regulations. We protect the confidentiality of PIFI and do not share it without your prior consent.
In general, PIFI includes all information of a personal nature supplied on account applications, questionnaires and other information provided in order to obtain accounts, obtain credit, enter into advisory or other relationships, etc. It includes a person’s first and last name (or first initial and last name) and one of the following: social security number, driver’s license number or state issued identification card number, or a financial account number, credit card number, or debit card number, with or without any required security code, access code, personal identification number or password that would permit access to a resident’s financial account. PIFI does not include information that we have taken steps to verify and reasonably believes could lawfully be obtained from federal, state or local government records, widely distributed media (telephone book, television or radio program) or disclosures to the general public required to be made by federal state or local law. PIFI that can be lawfully obtained from a website available to the general public is not protected, even though the website operator requires a fee or password.
The Health Insurance Portability and Accountability Act (“HIPAA) establishes national standards for privacy and security of employee health data, governing the use and protection of “Protected Health Information” (“PHI”). Privacy rules issued by the US Department of Labor are designed to protect PHI, including name, dates, zip code, telephone numbers , social security numbers, medical record numbers, health plan numbers, license numbers, photographs and information shared with other providers.
FIRM NAME AS A POLICY DOES NOT DISCLOSE ANY CONSUMER OR CUSTOMER NON-PUBLIC INFORMATION TO NON-RELATED THIRD PARTIES OTHER THAN IN CONTROLLED CIRCUMSTANCES AS SPECIFICALLY ALLOWED BY REGULATION S-P.
Information we collect automatically on our website. HealthView Services may collect certain information automatically (either directly, through the use of cookies and Web beacons or through the use of third-party service providers, agents or independent contractors who help us maintain our websites and provide other administrative services to us).
This information includes:
- Your browser type and language, and operating system
- Your Internet Protocol (IP) address, which is the number automatically assigned to your computer whenever you access the Internet and that can sometimes be used to derive your general geographic area
- The times that you visit our website
- Information about areas of our website you visited
- The sites you visited immediately before and after visiting our website
- Web pages and advertisements you view and links you click on within our website
- Unique identifiers when using a mobile device, including mobile device identification numbers
- Information that can identify the physical location of your mobile device in accordance with applicable law and industry guidelines and best practices. We will seek your permission before we identify the physical location of your device and before we engage in any continuous tracking of your device’s location.
- Information collected through Cookies, Web beacons and other technologies
- A “Cookie” is a piece of information a website sends to your computer while you are viewing the website that allows the site to recognize your computer each time you visit. You have the choice to set your browser to accept all cookies, reject all cookies or notify you when a cookie is set. (Each browser is different, so check the “Help” menu of your browser to learn how to change your cookie preferences.) It is up to you whether to allow us to send you cookies. However, if you do not accept cookies from us, your ability to use some areas of our site will be limited, such as our tools and other online features.
- Web beacons (sometimes called transparent GIFs, clear GIFs or Web bugs) are small strings of code that provide a way for us to deliver a small graphic image (usually invisible) on a Web page or in an email. Web beacons can recognize certain types of information on your computer such as cookies, the time and date a page is viewed, and a description of the page where the Web beacon is placed.
- Information about your interactions with email messages, such as the links clicked on and whether the messages were opened or forwarded
- Upon request for assistance, your bandwidth speed and information about the software programs installed on your computer
- Standard server log information
Information you provide directly to HealthView Services through our website. You are not required to provide information about yourself when you visit our website. However, in order to help us better serve you, we may ask you for information when you register for certain services, access various content or features, or directly contact us through our website, including:
- Contact information, such as name, email address, postal address, phone number, fax number and mobile number
- Payment information
- User name, password, and password-reminder questions and answers
- Age and date of birth
- Other demographic information, such as gender, employment information and ZIP code
- Information for a public profile, such as your interests, your profile photo, personal updates and descriptive information that you provide, and the address of your Facebook page, LinkedIn profile, Twitter, Pinterest or your website
- Communications preferences, such as which emails and newsletters you would like to receive
- Search queries
- Contact information about others when you provide information about others or refer a friend to a particular site or service
- Correspondence you send to us
- Information collected about you offline (including via fax, mail and in person)
- Other information that may help us to provide customized content or advertising
Information we obtain from other sources. We may collect information about you from other sources, including through interactive applications (such as mobile devices and third-party services), from Service Providers, Licensed Providers and from commercially available sources (such as data aggregators and public databases). The following are examples of information we may collect from other sources:
- Name, postal address and telephone number
- Demographic data, such as age, gender and income level
- If you access third-party services (such as Facebook Connect) through our website, your username and password for those services and other information available about you on those services
- Your interests and purchase behavior
- Publicly observed data, such as activities on blogs, videos and other online postings
- Other navigation data, such as websites visited and advertisements viewed or clicked on
Please note that all of the information we collect about you may be combined, and it may be used to help us tailor our communications to you and may be shared with third parties consistent with this Privacy Policy.
How Is It Protected?
With certain exceptions, we may not disclose PIFI of any Consumer to any NTP without prior notice and consent by the Consumer. An NTP is any person, firm or corporation that is not controlled by, controlling or under common control with HealthView Services.
Exceptions
There are Exceptions that allow us to disclose PIFI to persons or firms that are NTPs without prior permission. The disclosure of PIFI under any of the circumstances set forth below shall only be made by HealthView Services after review and approval by the CCO. The exceptions are as follows:
- The PIFI is necessary to service or process a transaction or a financial product or service requested or authorized by the Consumer or Customer. The NTP must be under contract with HealthView Services designed to ensure that the NTP will (1) maintain confidentiality of the information to the same degree as HealthView Services and (2) will use the information solely for the purposes disclosed;
- The PIFI is necessary to provide some ancillary recording or reporting of the transaction, product or service, account maintenance, confirmation, accruing fees or bonuses, etc.;
- The NTP has a legal or beneficial interest relating to the Consumer or Customer or is acting in a fiduciary or representative capacity;
- The disclosure is for the purpose of protecting the confidentiality or security of HealthView Services’s records, or to prevent potential or actual fraud, unauthorized transactions, claims or other liability, for institutional risk control or resolving customer disputes or inquiries;
- The disclosure is to provide information to insurance rate advisory organizations, guaranty funds or agencies, agencies rating HealthView Services, assessing compliance with industry standards, attorneys, accountants and auditors of HealthView Services;
- Subject to the Right to Financial Privacy Act, the disclosure is to a law enforcement agency, regulator, self-regulatory agency or in a public safety investigation;
- The disclosure is to a consumer reporting agency in accordance with the Fair Credit Reporting Act, or from a consumer report prepared by such an agency;
- The disclosure is in connection with a proposed sale, merger or transfer of a business unit, limited to PIFI about Customers of such unit;
- The disclosure is to comply with federal, state or local laws, rules or regulations, including requirements of self-regulatory organizations or a subpoena or judicial process; or
- The PIFI is provided under a contract with the NTP (a) requiring the NTP to maintain the same level of confidentiality as is required of HealthView Services and (b) limits any exceptions to those listed above.
Notice Requirements
We must provide an Initial Privacy Notice to (a) every customer at all times and (b) every consumer where we intend to disclose that Consumer’s PIFI to any NTP under any non-exempt circumstances. Each recipient must also have been provided with a “reasonable” time to “opt out” or not (see below).
The Initial Privacy Notice must be provided to the Customer, with certain exceptions, AT OR BEFORE the time we establish the Customer relationship and to the Consumer BEFORE we make any disclosures of that Consumer’s PIFI to a NTP. The Initial Privacy Notice may be provided in written or electronic form (if the Consumer or Customer is able to acknowledge receipt electronically).
We Share Your Information
HealthView Services May Share Your Information With:
- Service Providers. Service Providers are third parties with whom we may work to offer a service or feature, such as LinkedIn. You can tell when you are accessing a service offered by a Service Provider because the Service Provider’s name will be featured prominently. You may be asked to provide information about yourself to register for a service offered by a Service Provider. In doing so, you may be providing your information to both us and the Service Provider, or we may share your information with the Service Provider. Please note that the Service Provider’s privacy policy may apply to its use of your information.
- Licensed Providers. Licensed providers are third parties who make available special discounts, products or services.. Licensed providers are required to keep HealthView Services member information confidential, and to use the information only to offer the contracted products or services to HealthView Services and HealthView Services members.
- Corporate Affiliates. We may share information including your personally identifiable information with wholly or majority owned affiliates or other affiliated organizations, such as Healthview Advisory Services, Inc. (“HVAS”) so they can provide you with information about services and programs that might interest you.
- Approved Vendors. We may share your information including your personally identifiable information with companies we hire to provide certain administrative services such as processing address labels, managing databases and sending mailings.
- Selected Nonprofit Organizations. We may occasionally engage in “list exchanges” with selected nonprofit organizations in which we may share your information including your personally identifiable information.
- Aggregate Data. We may share aggregate statistics and other nonpersonally identifiable information with the media, government agencies, advertisers and other third parties. These aggregate statistics will not allow anyone to identify your name or other personally identifiable information.
- HealthView Services May Share Your Information With Other Third Parties:
- Pursuant to Legal Requirements. HealthView Services will disclose your personally identifiable information in response to a subpoena or similar investigative demand, a court order, or a request for cooperation from a law enforcement agency or other government agency; to establish or exercise our legal rights; to defend against legal claims; or as otherwise required by law.
- To Protect Our Website and Users. We will disclose your personally identifiable information when we believe disclosure is necessary to investigate, prevent or take action regarding illegal activity, suspected fraud or other wrongdoing; to protect and defend the rights, property or safety of our company, our employees, our website users or others; or to enforce our website terms and conditions or other agreements or policies.
- In Connection With Business Transfers. We may share your personally identifiable information as required by law, such as in response to a subpoena or court order or in the unlikely event of a substantial corporate transaction, such as the sale of HealthView Services or our affiliates, a merger, consolidation, asset sale or bankruptcy.
We May Share With Third Parties
We may allow third-party analytics companies, research companies or ad networks to collect non-personal identifiable information on our website. These companies may use tracking technologies, including cookies and Web beacons, to collect information about users of our site in order to analyze, report on or customize advertising on our site or on other sites. We may provide links or automatically redirect you to third-party websites that do not operate under this Privacy Policy. For example, if you click on an advertisement presented on our site, you may be taken to a website that HealthView Services does not operate or control. You should review the privacy policies and terms of use of all third-party websites you access. These third-party websites may independently solicit and collect information from you. On occasion, those third-party websites may provide us with information about your use of those sites. PLEASE NOTE THAT THIS PRIVACY POLICY DOES NOT COVER THE COLLECTION AND USE OF INFORMATION BY SUCH THIRD PARTIES.

How We Keep Your Information Secure
HealthView Services uses commercially reasonable physical, procedural and technological security measures to help prevent unauthorized access to and improper use of the information we collect about you. For example, only authorized employees and authorized third parties are permitted to access personally identifiable information, and they may do so only for lawful purposes. When we share aggregate data with third parties we prohibit them from reengineering nonpersonal data to permit identification of individual users. We maintain physical, electronic and procedural safeguards that comply with federal standards to guard your nonpublic personal information. However, no website or Internet network can be completely secure. Although we take steps to secure your information, we do not guarantee, and you should not expect, that your personal information, searches or other communications will always remain secure. Please refer to the U.S. Federal Trade Commission’s website for information on how to protect yourself from identity theft.
How You Can Keep More of Your Information Private
You have options for limiting who can see your information:
- If you choose to share contact information with us, you will have the opportunity to update or delete that information or to shut down your account
- You may request that we not share personally identifiable information about you with Licensed Providers and Selected Nonprofits at any time
- You may request to stop receiving communications from us by clicking here. Please note that if you opt out of receiving communications from us, you may continue to receive material from Service Providers, and/or other parties to whom we provided your information before processing your request to opt-out of communications. You will need to directly contact those third parties in order to opt out of receiving such communications. By joining HealthView Services, taking advantage of certain member benefits, participating in HealthView Services activities or visiting our website, you accept the practices described in this Privacy Policy. HealthView Services’s policies, content, information, disclosures, disclaimers and features may be revised, modified, updated and/or supplemented at any time and without prior notice at the sole and absolute discretion of HealthView Services. If this Privacy Policy is revised the new policy will be posted on our website and we will revise the “last updated” date at the top of this policy. If we make any material changes in the way we collect, use and/or share personal information that may impact you, we will notify you by sending an email to the email address you most recently provided to us (unless we do not have an email address for you) and/or by prominently posting notice of the changes on our website.
